Conduct in-home assessments on health plan members.
Perform assessments that average 45-60 minutes in length, completed electronically.
Review past medical history and conduct a review of systems.
Perform medication reviews and evidence-based practice screenings.
Check vitals and conduct physical exams including point of care testing as appropriate.
Identify diagnoses to be used in care management and active medical management.
Formulate lists of current and past medical conditions based on clinical knowledge and assessment findings.
Communicate assessment findings to inform member’s primary care provider of potential gaps in care.
Identify needs and opportunities for clinical referrals.
Identify urgent and emergent situations and intervene appropriately.
Educate members on disease processes, medications, and compliance.

NP license and national certification (ANCC or AANP)
Family, Geriatrics, Adult, Adult-Gerontology Primary or Acute Care certification
1+ years of experience as an NP in a clinical setting
Proficient with electronic medical records
Excellent administrative, organizational, and verbal skills
