Job Responsibilities
- Act as liaison between patients, physicians, and the respective care team.
- Coordinate care across the continuum.
- Provide education and information support for patients.
- Facilitate decision-making and advocacy.
- Monitor patient outcomes and participate in quality improvement activities.
- Collaborate with health care team members to positively impact patient outcomes and experiences.
Required Skills
Core Skills
- Knowledge of HEDIS and Star ratings
- Risk adjustment and HCC coding
- Proficient in computer applications
- Typing skills
- Experience in Electronic Medical Record systems
Preferred Skills
- Bilingual
- Preferred certification in area of expertise
Requirements
Experience: 3 Years of clinical experience working in a hospital, managed care environment or medical office.
Education: Bachelor Science Nursing – BSN required.
License & Certification:
- Active Registered Nurse license
- Neonatal Resuscitation Program
- Pediatric Advanced Life Support
- Basic Life Support
- Advanced Cardiovascular Life Support
