Job Responsibilities

Provides case finding, consultation, health-focused assessment/care coordination services and care planning services to adults with disabilities, older adults, and their families.
Provides services in home and community-based settings to preserve health, maintenance of function, and continued independence.
Assesses needs, monitors quality of care, and provides education, advocacy, and support.
Provides nursing consultation and guidance to departmental staff.

Requirements

Experience: One year of clinical nursing experience required (preferably with geriatric populations) with at least 1,000 hours in an acute care hospital or 2,000 hours in a home health setting.

Education: Bachelor's degree in nursing (BSN) required; Associate degree in nursing (ADN) will be considered if enrolled in a BSN program with degree completion within 2 years of date of hire.

License & Certification:

Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN)
Current Basic Life Support Provider (BLS) issued by American Heart Association
