Job Responsibilities
- Monitor the health and wellbeing of residents.
- Provide leadership support and assistance with resident care and services.
- Administer medication care.
- Communicate with resident support services and families.
- Maintain clinical quality assurance.
- Lead, demonstrate, and support wellness elements core to Phoenix programs and services.
- Identify ongoing needs and services of residents through the assessment/Personalized Service Plan (PSP) process.
- Ensure proper follow-through and documentation for residents with changes in clinical care needs.
- Assist the Wellness Director with clinical sections of monthly wellness visits and PSP.
- Inform the Wellness Director and/or Executive Director of changes in medication or service levels.
- Assist with obtaining weights and vital signs monthly for each resident.
- Assist with direct resident care as needed.
Required Skills
Core Skills
- Licensed Practical Nurse or Registered Nurse certification
- Leadership capabilities
- Knowledge in Assisted Living industry
- Knowledge in Dementia care or Skilled Nursing
Preferred Skills
- Minimum of one year of experience working with elders or disabled individuals
- Ability to work weekends and evenings
