Job Responsibilities
- Provides direct patient care as defined in State's Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s).
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.
Required Skills
Core Skills
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
- Good verbal and written communication skills.
- Organizational skills.
Preferred Skills
- Three years nursing experience preferred.
- Community/home health or medical/surgical experience preferred.
- Ability to drive and/or have access to transportation for agency related travel.
- Possesses and maintains current CPR certification.
Requirements
Experience: Three years nursing experience preferred.
Education: Completed a requisite practical (vocational) nursing program.
License & Certification:
- Licensed practical nurse or licensed vocational nurse certification.
- Current CPR certification.
