Job Responsibilities
- Conduct complete assessments and establish appropriate plans for patient care.
- Initiate interventions within desired timeframes.
- Collaborate with patients, families, and healthcare teams to achieve care goals.
- Utilize patient/family strengths in the problem-solving process.
- Provide continuity of care and discharge planning services compliant with regulatory standards.
- Participate in communication processes for smooth transitions of care.
- Provide case management services related to health care, finances, housing, and family dynamics.
- Educate patients and families regarding access to community resources and services.
- Document discharge planning interventions and utilization review activities in a timely manner.
- Perform concurrent and retrospective reviews based on department standards.
- Communicate effectively with the healthcare team.
- Work with Social Work and support personnel to implement safe plans of care.
- Serve as an active member of the Outcome Facilitation Team.
- Identify and resolve barriers to discharge with medical staff and departments.
- Refer cases to physician advisors for efficient progression of care.
- Remain knowledgeable about healthcare regulations, reimbursement issues, and community resources.
- Conduct UM activities including clinical updates and managing delays.
- Deliver CMS regulatory notices within required timeframes.
- Maintain relationships with community-based agencies and networks.
- Collaborate with Ambulatory Care Management to meet patient outcomes.
- Serve as an educator regarding admission status and regulatory requirements.
Required Skills

- Basic knowledge of MS Office products
- Knowledge of Medicare A and B guidelines

- 2 years of clinical nursing experience

Requirements
Experience: 2 years of clinical nursing experience
Education: Bachelor’s Degree in Nursing
License & Certification:
- Registered Nurse License issued by the state of practice
