Job Responsibilities
- Report directly to the Administrator, Executive Director or Patient Care Manager Senior.
- Function under the direction, instruction and supervision of the Registered Nurse and/or Clinical Supervisor.
- Help the patient to maintain good personal hygiene.
- Assist in maintaining a safe and healthy environment.
- Prepare and serve light meals.
- Assist in feeding patient when needed.
- Assist the patient in ambulation.
- Assist with the safe use of a walker or wheelchair as instructed by the nurse.
- Assist with light housekeeping tasks such as emptying trash and changing linens.
- Assist with certain treatments as ordered on the aide plan of care.
- Promote patient's mental alertness through involvement in activities of interest.
- Provide basic emotional and psychological support to the patient and household members.
- Report any change in the patient's mental or physical condition to the Registered Nurse.
- Demonstrate basic care to the primary caregiver.
- Practice infection control measures and standard precautions.
- Provide accurate documentation with visit itineraries.
- Attend staff meetings.
- Participate in at least one Quality Improvement activity yearly.
- Complete documentation in a timely manner.
Required Skills
Core Skills
- Compassionate, kind, and empathetic nature.
- Ability to provide care in various home environments.
- Flexibility and composure in crisis situations.
- Certified Nursing Assistant (CNA) license.
Preferred Skills
- Six (6) months of patient care experience in hospice, home health, ICU, geriatric, or oncology.
- Preferred experience in hospital settings such as RN, Med Surg, PACU, ER, critical care, or other nursing areas.
Requirements
Experience: Six (6) months of patient care experience preferred.
Education:
License & Certification:
- Certified Nursing Assistant (CNA) license.
- Current driver's license and automobile insurance.
- Current CPR certification.
