Job Responsibilities
- Provide hands-on care in the home for a group of patients using the nursing process and clinical standards.
- Oversee the activities of the multi-disciplinary team that provides care to the patient.
- Monitor the provision of therapeutic services through written and verbal communication with all disciplines.
- Ensure that all visits scheduled and provided are authorized and covered by physician orders.
- Review patient visits weekly and provide plan to Patient Care Supervisor and team assistant, including anticipated weekend visits.
- Inform the scheduler of any schedule changes and communicate caseload and patient care issues to Home Care Supervisor.

Requirements
Experience: 1 year Registered Nurse or 1 year Licensed Practical Nurse
Education: Associate's or Diploma in Nursing; Preferred Bachelor's in Nursing
License & Certification:
- BLS (Basic Life Support)
- RN (Registered Nurse) License
- Driver's License State of Florida
Schedule:
- Occasional weekend work
- On-call responsibilities
