Job Responsibilities
- Provide appropriate supervision of the people supported by managing crisis situations and adhering to policies, procedures, and regulations.
- Maintain a clean, safe, and organized therapeutic environment.
- Implement person-centered, individualized plans.
- Assist with personal care and activities of daily living (ADLs) including bathing, dressing, and toileting.
- Handle laundry and housekeeping tasks.
- Administer medication as prescribed in accordance with applicable guidelines.
- Teach and develop skills.
- Support person-centered advocacy for the individuals.
- Assist in communication, self-expression, and resolve disputes among individuals.
- Facilitate ethical decision-making.
- Plan and prepare balanced meals and snacks in accordance with dietary restrictions of specific individuals.
- Transport and escort individuals for recreational activities, medical appointments, and home visits.
- Assist individuals with making purchases and safeguarding personal allowance funds.
- Provide additional supports for individuals with complex medical needs under the supervision of an RN or LPN.
- Assist with transfers, turns, and lifts using appropriate body mechanics.
- Gather and stock medical supplies.
- Measure and document food and liquid intake and urinary and fecal output, reporting changes to medical staff.
- Check vital signs such as temperature, blood pressure, and heart rate.
- Schedule and follow up on appointments in coordination with the Health Care Specialist.
- Perform catheter care and assist with the care of ostomies.
- Provide enteral nutrition and take care of wounds as per medical provider's orders.
- Maintain a clean environment by managing sharps, bodily waste, and cleaning rooms.
- Collect, record, and summarize observable behavior data on individuals.
- Observe and report changes in behavior or other signs to the clinical team.
- Complete and maintain all necessary trainings and certifications.
- Review communication logs and ensure compliance with required paperwork and documentation.
- Use foundational teaching skills to implement training and progressive teaching techniques.
- Meet and demonstrate OPWDD Core Competencies.
- Comply with all relevant regulatory requirements including HIPAA.
- Perform additional duties as assigned.
Required Skills
Core Skills
- Current/active certification as a Certified Nursing Assistant (CNA) in New York.
- Effective written and oral communication skills.
- Ability to document services for billing and tracking.
- Capability to meet physical demands such as lifting and moving individuals.
- Ability to assist with various medical interventions.
Preferred Skills
- Hold or obtain certifications for crisis intervention, medication administration, CPR, and First Aid.
- Certifications related to specific medical conditions as required.
