Job Responsibilities
- Deliver comprehensive, evidence-based primary care for patients
- Diagnose and manage acute and chronic conditions
- Promote wellness through preventive care and patient education
- Maintain accurate and timely clinical documentation
- Collaborate with physicians to ensure high-quality, coordinated care
Required Skills
Core Skills
- Current, unrestricted Illinois APRN license
- Board Certification in Family Medicine
- Active DEA registration
- Illinois Controlled Substance License
- EMR proficiency (Epic preferred)
- Strong documentation and organizational skills
Preferred Skills
- Minimum 3 years of clinical experience preferred
- Strong communication skills
- Commitment to patient-centered, community-focused care
Requirements
Experience: Minimum 3 years of clinical experience preferred; new graduates will be considered
Education:
License & Certification:
- Current, unrestricted Illinois APRN license
- Board Certification in Family Medicine
- Active DEA registration
- Illinois Controlled Substance License
Schedule:
- 5-day work week including one evening per week
- One Saturday a month (off on the Friday worked)
- Weekend call rotation, one weekend per month
