Lead the development and oversight of individualized care plans, ensuring compliance with Colorado Medicaid regulations, federal regulations and agency policies.
Conduct initial and ongoing client assessments, including care plan updates after condition changes or as requested by clients, families, or case managers.
Serve as the primary liaison for clients, families, and caregivers, addressing care coordination and adjustments with clarity and compassion.
Ensure compliance with all applicable state and federal home care regulations, including EVV requirements and documentation audits.
Oversee Attendant supervision, including in-home visits, skills validation, and support with challenging client care scenarios.
Develop and implement caregiver training programs that prioritize safety, professionalism, and confidentiality.
Monitor client safety through risk assessments, incident investigation, and corrective action planning.
Partner with agency leadership on strategic planning, program development, and building a client-centered care culture.

Minimum of 3 years of experience in healthcare, home care, or related settings
Experience with Colorado's In-Home Support Services (IHSS) program
Familiarity with Colorado’s Home and Community-Based Services (HCBS) regulations
Experience with caregiver training and supervision
