Pay: $42.44 - $74.57, $15,000 Hiring Bonus for eligible external hires, and Relocation Assistance available.

Job Responsibilities
- Provide professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment.
- Conduct assessment and planning.
- Coordinate care and manage resource utilization.
- Engage in discharge planning.
- Document interventions.
- Ensure regulatory compliance.
- Act as a patient advocate.
Required Skills
Core Skills
- Acute care experience in a Medical Surgical or Inpatient setting
- Utilization Review competency
- Assessment and planning
- Care coordination
Preferred Skills
- Bachelor's Degree in Nursing or higher
- National Certification in area of specialty
- Experience in care management or utilization review
- Successful completion of TIPS program or Case Management Orientation Program
Requirements
Experience: 2 years of acute care experience
Education: Associate's Degree in Nursing upon hire.
License & Certification:
- Alaska Registered Nurse License upon hire
