Job Responsibilities
- Support daily patient flow and ongoing case management of an assigned panel of patients.
- Coordinate with an interdisciplinary team including patients, families, physicians, medical assistants, social workers, counselors, and support staff.
- Advocate and support the case management process.
- Match ongoing needs of individuals with appropriate medical, psychological, health, or social care/services.
Required Skills
Core Skills
- Professional nursing experience
- Nurse case management experience
- Strong communication and coordination abilities
Preferred Skills
- Prior clinic or outpatient experience
- Supervisory experience
Requirements
Experience: 2 years Professional nursing experience, 2 years Nurse Case Management experience
Education: Bachelor's Degree in Nursing
License & Certification:
- Alaska Registered Nurse License
