Job Responsibilities
- Responsible and accountable for prescribing, delegating and coordinating patient care.
- Uses clinical judgment to assess the patient's situation and develop an appropriate plan of care.
- Ensures patient care meets standards of safety, effectiveness, and patient rights.
- Oversees care delivered by patient care team and coordinates the plan of care.
- Provides education and facilitates learning for patients and families with sensitivity to diversity.
- Collaborates with healthcare professionals to develop and implement patient care plans.
- Advocates for the patient and identifies ethical and clinical concerns.
- Continuously evaluates the patient's condition and implements treatment changes as necessary.
- Maintains knowledge of resources available to manage patient care.
- Completes and analyzes comprehensive assessments during patient intake.
- Coordinates treatment plans including payors, supplies, and community resources.
- Ensures appropriate sequencing of consults, tests, and procedures.
- Utilizes clinical pathways for effective disease management.
- Assesses educational needs of patients considering barriers to care.
- Coaches patients and their families towards successful self-management.
- Facilitates communication with Providers and the healthcare team.
- Identifies and addresses barriers to service or treatment.
- Evaluates the patient's response to treatment regimens.
- Leads discharge planning to ensure timely and safe transitions to the next level of care.
- Communicates effectively to resolve conflicts with the healthcare team, patients, and families.
- Works to improve quality and reduce treatment delays.
- Provides consultation and training for medical staff regarding discharge planning.
- Participates in community events and educational speaking engagements.

Requirements
Experience: At least 4 years in Pediatric nursing, Case Management, Care Coordination, Utilization Review, or Community-based nursing required.
Education: BSN
License & Certification:
- Registered Nurse in the State of Texas Upon Hire
- Accredited Case Manager (ACM) preferred
- Certified Case Manager (CCM) preferred
- Care Coordination and Transition Management (CCTM) preferred
- Basic Life Support for Healthcare Providers required
