Job Responsibilities

  • Perform systematic, comprehensive interviews to determine the urgency of patient problems during phone triage.
  • Accurately document data for all calls, including patient demographic information, chief complaints, recommendations for home treatment, and referrals.
  • Perform and document follow-up calls with patients as necessary.
  • Act as a liaison between patients and providers, answering inquiries regarding health issues.
  • Serve as an advocate for patients for internal and external referrals.
  • Make referrals to the appropriate level of care within required timeframes.
  • Elicit questions and concerns from patients to assess symptoms and create treatment plans.
  • Coordinate medical care and schedule patient follow-up appointments as instructed.
  • Provide professional advice, education, and options based on assessments and clinical judgment.
  • Advise patients to seek medical care at emergency rooms when appropriate.
  • Assist callers with referrals and scheduled clinical appointments.
  • Evaluate patient understanding of care plans and provide health education as needed.
  • Document all calls, responses, and referrals, ensuring compliance with regulatory bodies.
  • Perform follow-up calls to evaluate care effectiveness.
  • Maintain accurate patient documentation according to regulatory requirements.
  • Provide counseling, home treatment advice, and crisis intervention as per established protocols.
  • Perform other duties as assigned based on departmental needs.

Preferred Skills

  • Call center experience
  • Specialty Nursing Certification

Requirements

Experience: 5 or more years of clinical nursing experience in Emergency, Pediatrics, Obstetrics, Behavioral Health, or Critical Care; 1 or more years in triage or health coaching can substitute for clinical experience.

Education: High School Graduate or GED; Bachelor Degree in Nursing

License & Certification:

  • Basic Life Support Heartsaver (BLS HS) - American Heart Association (AHA) within 14 days
  • Licensed Registered Nurse (LRN) in good standing with a multistate or single state license
