Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts initial assessments, re-assessments, and focused assessments based on patient condition.
- Identifies problems based on clinical judgment about patient’s response to health conditions or needs.
- Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Forms plans of care and coordinates transitions or discharge planning.
- Implements nursing care according to the care plan to ensure continuity during hospitalization and preparation for discharge.
- Ensures patient safety through medication administration and provision of ordered treatments.
- Educates patients and families.
- Documents nursing care in healthcare records.
- Continuously evaluates patient status and effectiveness of nursing care, modifying the care plan as needed.
- Coordinates care actions with patients, families, and healthcare team members to manage patient care from admission to discharge.
- Delegates and supervises nursing tasks as required.
- Prioritizes patient care.

Requirements
Experience: None required
Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS) - American Heart Association
- Advanced Cardiac Life Support (ACLS) - Preferred
- Pediatric Advanced Life Support (PALS) - Preferred
- Neonatal Resuscitation Program (NRP) - Preferred
