Job Responsibilities
- Directs and supervises activities of the neighborhoods for the provision of nursing care services
- Makes rounds in the entire health center at a minimum of twice per shift; monitoring for resident safety and infection prevention and assessing resident care
- Prepares narrative and statistical reports documenting unit activities as required/requested by Nursing Administration; monitors completion of nursing documentation for the shift
- Conducts audits as assigned
- Supports the WEL mission, objectives, and administrative policies
- Demonstrates knowledge of nursing practice standards in directing resident care activities
- Assists in the development and implementation of nursing department policies and programs
- Ensures adequate staff coverage to meet proper ratios, PPD, and acuity of residents according to daily census
- Confers with residents and their families; makes referrals to other departments when appropriate
- Communicates status of residents to oncoming personnel; communicates with physicians; transcribes, clarifies, and implements physicians’ orders as needed
- Audits medical records, including assessments, changes in condition, incidents, etc. and document as required
- Coordinates incident investigations, gathers data, assesses information, communicates in writing the results of the investigation, outcomes and makes recommendations for the plan of action
- Completes and audits resident admission including the development of care plan; attends and participates in the Plan of Care meetings as needed
- Responds to emergencies; assesses the condition of the resident; and formulates a correct plan of action
Required Skills
Core Skills
- Current active RN license
- Current CPR certification
- Ability to listen and understand spoken information
- Ability to communicate information and ideas effectively
Preferred Skills
- Prior experience of six months providing care in a SNF or hospital
