Job Responsibilities

Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Collects and analyzes data about a patient, including physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
Identifies problems and clinical judgments about the patient’s response to health conditions or needs.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Implements nursing care according to the care plan.
Ensures continuity of care during hospitalization and in preparation for discharge.
Documents care in the patient’s record.
Ensures patient safety and the administration of medications and treatments.
Provides patient and family education.
Continuously evaluates the patient’s status and the effectiveness of nursing care.
Coordinates care with patients, families, and/or members of the healthcare team.
Delegates, supervises, and prioritizes nursing tasks.

Experience:

Education: Graduation from an accredited School of Nursing

License & Certification:

Valid current Arizona Nursing License or compact state license
Basic Life Support (BLS) - American Heart Association
