Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Collects and analyzes data about a patient, including physiological, psychological, sociocultural, spiritual, economic, and life-style factors.
Conducts initial assessments, re-assessments, and focused assessments based on patient condition.
Identifies patient problems and needs through clinical judgment.
Plans measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Forms a plan of care and ensures continuity of care.
Documents care provided in the patient's record.
Evaluates patient status and effectiveness of nursing care, modifying the care plan as needed.
Manages and corrects patient care coordination from admission to discharge.
Delegates effectively and supervises care as needed.
Prioritizes patient care tasks.
