Job Responsibilities

Provide therapeutic patient and family centered care.
Collect and assess data related to patient health status to develop and evaluate the plan of care.
Utilize evidence to drive clinical decisions and practice related to individual and population health.
Establish goals and strategies for care coordination needs across settings.
Consider factors related to safety, effectiveness, and fiscal accountability in planning and delivering care.
Delegate and supervise care in accordance with the WI Nurse Practice.
Participate in continuous quality improvement initiatives.
Perform/direct ACLS.
Conduct independent rhythm interpretation and defibrillation.
Administer advanced lifesaving medications.
Conduct independent, templated visits for patient assessment based on established protocols.
Provide specialty patient education based on required certifications.
Assist with complex procedures and advanced wound therapy.
Manage central line maintenance and device interrogation.
Coordinate patient care, perform telephone triage/eVisits, and assist with minor procedures.

Required Skills

Experience in patient education
Complex infusion management skills
Certification in sedation

Requirements

Experience: At least 1 year of relevant nursing experience preferred.

Education: Bachelor's Degree in Nursing preferred.

License & Certification:

Current State of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating NLC state.
AHA Basic Life Support for the Healthcare Provider (BLS) required within 90 days of hire.
