Job Responsibilities

Collaboratively work as an active member of the interprofessional team to provide therapeutic patient and family centered care.
Systematically and continuously collect and assess data related to patient health status.
Develop, execute, and evaluate the plan of care.
Utilize evidence to drive clinical decisions and practice related to individual and population health.
Establish goals and strategies for meeting care coordination needs with the patient, family, and/or other care providers.
Consider factors related to safety, effectiveness, and fiscal accountability in planning and delivering care.
Delegate and supervise care in accordance with the WI Nurse Practice.
Participate in continuous quality improvement initiatives.
Assume responsibility for professional development.
Perform/direct ACLS.
Conduct independent rhythm interpretation and defibrillation.
Administer advanced lifesaving medications.
Conduct independent, templated ('RN-only') visits for patient assessment.
Provide specialty patient education where certification is required.
Assist with complex procedures.
Provide advanced wound therapy.
Manage central lines and TPA administration.
Interpret device interrogation and take action.
Coordinate patient care, provide direct patient care, education, telephone triage/eVisits, and assist with minor procedures.

Required Skills

Core Skills

Nursing licensure in Wisconsin or Multi-state Nursing License
ACLS certification
Ability to perform independent patient assessments and make clinical decisions

Preferred Skills

Bachelor's Degree in Nursing
Clinical experience in patient population management

Requirements

Experience: At least 1 year of relevant nursing experience preferred.

Education: Nursing licensure is required; Bachelor's Degree in Nursing preferred.

License & Certification:

Current state of Wisconsin Registered Nurse License
American Heart Association Basic Life Support for the Healthcare Provider (BLS)
