Job Responsibilities

  • Utilize the Nursing Process of assessing, planning, implementing, and evaluating patient care.
  • Manage assigned personnel and promote collaboration among the multi-disciplinary health care team.
  • Assess and document the physical, psycho-social, growth, Functional Independence Measures (FIM) scores, and developmental dimensions of the patient.
  • Include patients and other health care team members in developing and implementing the plan of care using medical/nursing diagnoses.
  • Customize and prioritize the plan of care and help develop a problem list as needed.
  • Evaluate long and short-term goals including communication of plan of care to patients, families, and providers as appropriate.
  • Conduct teach-back sessions with patients to facilitate outcome improvements.
  • Elicit information using effective counseling techniques and identify factors affecting communication or learning.
  • Conduct comprehensive holistic wellness and illness assessments and employ evidence-based clinical practice guidelines.
  • Provide leadership for revised diagnoses and care plans to improve patient outcomes.
  • Develop evidence-based clinical interventions and systems to achieve patient and system outcomes.
  • Comply with regulatory guidelines and professional practice standards.
  • Assist with data collection and educate staff on performance improvement indicators.
  • Ensure utilization of available safety equipment and processes for patient care.
  • Develop and implement care plans with other health care team members.
  • Act as a resource for professional communication with physicians regarding patient care.
  • Administer medications utilizing the 5 rights and available safety equipment.
  • Utilize clinical judgment with patient assessments and therapies to promote goal attainment.
