Job Responsibilities

  • Concurrently coordinates all activities and metrics related to disease specific care for Heart Failure (HF) and Stroke.
  • Collects, enters, analyzes, and presents Quality Improvement (QI) data and activities, including Core measure data, hospital and medical staff quality data, required measure data, complaint and occurrence data, and peer review data related to Disease Specific care.
  • Facilitates hospital-wide compliance with certification specific accreditation standards.
  • Champions performance improvement teams for disease specific care.
  • Provides staff education and serves as a disease specific resource for hospital leaders and staff.
  • Leads accreditation survey activities.
  • Provides Quality orientation for new employees.
  • Participates in interdisciplinary team rounds.
  • Demonstrates problem-solving skills and implements processes to improve patient care delivery.
  • Leads community outreach efforts related to disease specific care.

Requirements

  • Heart Failure Nurse Certification preferred
