Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessments, re-assessments, and focused assessments based on patient condition, setting, and population.
Identifies problems through clinical judgment about the patient's response to actual or potential health conditions or needs.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care to ensure continuity of care for the patient during hospitalization and preparation for discharge.
Documents nursing care provided in the patient's record.
Ensures patient safety and medication administration.
Provides education to patients and families.
Continuously evaluates patient status and effectiveness of nursing care.
Manages care coordination from admission to discharge.
