Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Collects and analyzes data about a patient as part of the assessment process.
Conducts initial assessments, re-assessments, and focused assessments based on patient condition.
Identifies problems based on clinical judgment about the patient’s response to actual or potential health conditions.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Documents assessment data, problem identification, and goals in the patient’s care plan.
Establishes daily goals with the patient and family.
Formulates a plan of care.
Implements nursing care according to the care plan.
Documents in the patient’s healthcare record.
Ensures patient safety.
Administers medication and provides ordered treatments.
Educates patients and their families.
Continuously evaluates patient’s status and the effectiveness of nursing care.
