Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial and focused assessments based on patient condition, setting, and population.
Identifies patient problems through clinical judgment.
Plans and sets measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Forms a plan of care and documents it in the patient’s care plan.
Implements nursing care according to the care plan and assures continuity of care.
Documents care provided in the patient’s record.
Continuously evaluates patient status and the effectiveness of nursing care, modifying the care plan as needed.
Coordinates care among patients, families, and healthcare team members.
Delegates, supervises, and prioritizes patient care.
