Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carry out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Collects and analyzes data about a patient to deliver nursing care.
Conducts initial, re-assessments, and focused assessments based on patient condition.
Identifies problems and sets measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family and formulates a plan of care.
Implements nursing care according to the care plan.
Ensures patient safety throughout care processes.
Administers medications and provisions of ordered treatments.
Educates patients and families regarding care.
Documents care in the healthcare record.
Evaluates patient's status and the effectiveness of nursing care continuously.
Coordinates care with patients, families, and healthcare team members.
Delegates and supervises nursing tasks.
Prioritizes patient care needs.

Experience:

Education: Graduation from an accredited School of Nursing
