Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts systematic, dynamic assessment of patient data including physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
Establishes daily goals with the patient and family.
Formulates a plan of care.
Implements nursing care according to the care plan.
Ensures patient safety and administers medications.
Provides ordered treatments and patient/family education.
Documents care in the healthcare record.
Continuously evaluates the patient’s status and the effectiveness of nursing care, modifying the care plan as needed.
Coordinates care management with patients, families, and healthcare team members from admission to discharge.
