Job Responsibilities

Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Collects and analyzes data about a patient as part of the nursing care process.
Conducts initial assessments, re-assessments, and focused assessments based on patient condition.
Identifies problems based on clinical judgment about the patient’s response to health conditions.
Sets measurable and achievable short- and long-range goals in collaboration with patient and family.
Documents assessment data, problem identification, and goals in the patient’s care plan.
Establishes daily goals with the patient and family.
Formulates a plan of care.
Implements nursing care according to the care plan.
Documents care provided in the healthcare record.
Evaluates patient status and the effectiveness of nursing care continuously.
Modifies the care plan as needed.
Coordinates care actions with patients, families, and healthcare team members to manage effective patient care.

Education: Graduation from an accredited School of Nursing

License & Certification:

Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days
Basic Life Support (BLS) - American Heart Association
