Job Responsibilities

Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial, re-assessment, and focused assessments based on patient condition, setting, and population.
Identifies problems and makes clinical judgments about patient responses to actual or potential health conditions.
Plans and sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care.
Implements nursing care according to the care plan during hospitalization and in preparation for discharge.
Documents care in the patient's record.
Evaluates patient status and the effectiveness of nursing care continuously, modifying the care plan as needed.
Coordinates care actions with patients, families, and/or members of the healthcare team from admission to discharge.
Delegates tasks and supervises team members.
Prioritizes care needs.

Education: Graduation from an accredited School of Nursing

License & Certification:

Valid current Arizona Nursing License or compact state license
Basic Life Support (BLS) - American Heart Association
