Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts systematic assessments including initial, re-assessment, and focused assessment.
- Identifies problems based on clinical judgment about the patient’s response to health conditions.
- Plans and sets measurable goals in collaboration with the patient and family.
- Implements nursing care according to the care plan ensuring continuity of care.
- Ensures patient safety through medication administration and provision of treatments.
- Educates patients and families regarding care.
- Documents nursing care in healthcare records.
- Continuously evaluates the patient’s status and modifies care plans as needed.
- Coordinates care actions from admission to discharge.
- Delegates and supervises care tasks as appropriate.

Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS) - American Heart Association
