Job Responsibilities

Assigned to a population of patients and provides individualized and comprehensive care to reduce barriers.
Coordinate diagnostic and treatment efforts to ensure prompt access to care.
Perform clinical assessments using the nursing process.
Provide education on disease process, plan of care, treatment modalities, and symptom management.
Collaborate and communicate effectively with the multidisciplinary team to coordinate patient care.
Ensure continuity of care and create an exceptional patient experience.
Initiate referrals to appropriate interdisciplinary teams for support through all stages of the continuum of care.
Provide coordination of care and support for patients, families, and caregivers.
Educate patient, caregiver, and family on disease process, plan of care, treatment, symptom management, and survivorship.
Demonstrate professionalism in the workplace.
Provide excellent customer service resulting in patient satisfaction.
Promote a patient-and family-centered care environment for ethical decision making and advocacy.
Welcome patients and family members warmly with introductions to self and team.
Communicate time for appointments, procedures, and potential delays.
Prioritize and triage incoming phone messages according to clinical needs.
Return calls within the same day and use positive service recovery techniques.
Return patient messages/requests within 24 hours.

Required Skills

Core Skills

ADN completion of accredited RN program
3 years of experience as an RN in inpatient or outpatient setting
AZ RN license
American Heart Association BLS certification
Computer skills and EMR documentation
