Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Collects and analyzes data about a patient as part of initial, re-assessment, and focused assessment.
Identifies problems and sets measurable, achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care.
Ensures continuity of care for the patient during hospitalization and in preparation for discharge.
Documents care in the patient's record.
Conducts continuous evaluation of patient status and the effectiveness of nursing care.
Manages patient care from admission to discharge through care coordination.

Education: Graduation from an accredited School of Nursing
