Job Responsibilities
- Perform visits and complete coordination of client care.
- Serve as the point of contact for all disciplines involved in patient care.
- Oversee the frequency of visits for the episode.
- Consult with the physician and office regarding patient care as needed.
- Perform accurate OASIS collection.
- Ensure the medication profile remains current.
- Ensure lab values have been reported to the physician in a timely manner.
- Attend weekly case conference and monthly case manager meetings.

- Current nursing practice knowledge and skills
- Ability to coordinate patient care
Preferred Skills
- One year of clinical experience
Requirements
Experience: Minimum of one year of clinical experience preferred.
Education:
License & Certification:
- Currently licensed as a Registered Nurse (RN) in the state of employment
- Valid state driver’s license and automobile liability insurance
