Job Responsibilities
- Assist branch leadership with maintaining a high performing hospice program.
- Manage, oversee, and direct patient care activities to ensure the delivery of quality patient care.
- Supervise, teach, evaluate, and develop associates.
- Ensure adherence to company policies and procedures and compliance with local, state, and federal regulations.
- Prepare for, serve on, and facilitate the Interdisciplinary Group (IDG) as assigned.
- Preside over IDG and present patients for review.
- Coordinate IDG meeting minutes and review patient charts for changes.
- Manage clinical and administrative recordkeeping functions.
- Document referrals, new physicians and facilities, DME pickup, and Medicare patient eligibility.
- Obtain authorization and reauthorization as needed and verify commercial insurance.
- Transfer patient documentation, satisfaction surveys, and monitor patient benefit periods.
- Direct the work of clinical associates including assigning admit and evaluation visits.
- Schedule visits at the beginning of benefit periods and ensure timely completion of visits.
Required Skills
Core Skills
- Current Registered Nurse licensure in state of residence
- Knowledge of hospice principles and practices
- Experience in hospice or home-based care setting
- Knowledge of managed care principles and regulatory guidelines.
Preferred Skills
- Bachelor’s degree in nursing
- Program management and oversight skills.
Requirements
Experience: At least three years’ experience of direct patient care and two years’ experience in hospice or home-based care setting.
Education: Graduate of accredited school of nursing; bachelor’s degree in nursing preferred.
License & Certification:
- Current Registered Nurse licensure in state of residence.
