Job Title: RN Clinical Manager, Home Health

Position: Clinical Manager overseeing direct care patient services

Job Type: Full Time

Pay: $77,200 - $106,200 per year

Shift: M-F / 8a-5p (ET) with rotating on-call expectations

Job Responsibilities

  • Coordinate and oversee all direct care patient services provided by clinical personnel.
  • Develop, plan, implement, analyze, and organize clinical operations for a managed location.
  • Conduct and delegate patient assessments and updates of care plans.
  • Manage the assignment of caregivers.
  • Oversee the delivery of care to all patients served by the location.
  • Receive case referrals and review patient information to determine needs.
  • Ensure patients meet admission criteria and make decisions to admit patients to service.
  • Assign appropriate clinicians to cases as needed.
  • Instruct and guide clinicians to promote effective care delivery.
  • Assist clinicians in establishing therapeutic goals and developing Plan of Care (POC).
  • Monitor cases for compliance with regulatory agencies and third-party payer requirements.
  • Coordinate communication between team members, physicians, and caregivers.
  • Establish location’s revenue and budget goals in consultation with the Branch Director.
  • Participate in sales and marketing initiatives.
  • Supervise all clinical employees assigned to a specific location.
  • Handle employee corrective actions and discipline issues fairly and objectively.
  • Participate in interviews, hiring, training, and development of direct care clinicians.
  • Evaluate performance relative to job goals and requirements.
  • Coach staff and recommend in-service education programs as needed.
  • Assess staff education needs based on clinical documentation and feedback.
  • Conduct regular staff education as needed based on assessment.
  • Analyze situations and evaluate alternative courses of action.
  • Review documentation of Case Managers and clinical staff for appropriateness of care.
  • Manage QA/PI activities and collaborate with Utilization Review staff for performance review.
  • Provide direct patient care on an infrequent basis during emergencies.
  • Act as Branch Director in the absence of the Branch Director.
  • Ensure compliance with Company standards and policies.
  • Participate in performance improvement activities and maintain clinical knowledge.
  • Maintain relationships with referral/community sources and participate in professional organizations.

Required Skills

Core Skills

  • Active and unrestricted Registered Nurse (RN) license
  • Management and people leadership
  • Home health experience

Preferred Skills

  • OASIS experience
  • OASIS certification (HCS-O, COQS, and/or COS-C)
  • Homecare Homebase (HCHB) experience
  • CMS PDGM billing knowledge or experience

Requirements

Experience: Two years as a Registered Nurse with at least one year of management experience in home care or hospice

Education: Associate degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred

License & Certification:

  • Active and unrestricted Registered Nurse (RN) license or Compact State RN license
  • Current CPR card and/or BLS certification

Schedule:

  • Normal hours of operation: M-F / 8a-5p (ET)
  • On-call expectation: Yes, rotating on-call shift
