Job Responsibilities
- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient.
- Confer with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment.
- Provide hands-on care, management, and evaluation of the care plan as per physician orders under Clinical Manager's supervision.
- Revise care plan in consultation with physician based on ongoing assessments.
- Coordinate appropriate care with various healthcare personnel, such as Physical Therapists and Occupational Therapists.
- Report patient care, condition, and progress to patient's physician and Clinical Manager continuously.
- Implement patient care plan in conjunction with patient and family.
- Coordinate, oversee, and supervise the work of Home Health Aides and Personal Care Workers.
- Monitor completion of documentation by home health aides and personal care workers.
- Discharge patients after consultation with the physician and Clinical Manager, preparing clinical documentation.
- Prepare appropriate medical documentation on all patients, including care plan changes.
Required Skills
Core Skills
- Strong med surg, ICU, ER, acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
- Reliability in using personal transportation
Preferred Skills
- Home Health experience
- Diploma, Associate or Bachelor's Degree in Nursing
Requirements
Experience: Minimum of one year nursing experience preferred
Education: Diploma, Associate or Bachelor Degree in Nursing
License & Certification:
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
