Position: Home Health & Hospice Nurse Case Manager

Job Type: Full-Time

Pay: $85K+ annually

Shift: Monday-Friday, 8:00 AM-5:00 PM

Conducting comprehensive physical, social, and psychological assessments.
Collaborating with patients, families, and the Interdisciplinary Group to create individualized care plans.
Managing care delivery and maintaining meticulous patient records.
Acting as a resource for families, making proactive referrals, and fostering a holistic care experience.

Experience: Previous Home Health or Hospice experience preferred
