Position: Registered Nurse Care Manager for Chronic Kidney Disease and End-Stage Renal Disease

Pay: $74,000.00-$90,000.00

Shift: Hybrid role with in-home visits and telecommunication

Craft personalized and thorough care plans addressing the clinical and non-clinical needs of each patient.
Conduct regular assessments onsite and virtually to detect undisclosed medical or social issues and changes in the home environment.
Initiate proactive outreach to patients and/or clinics to evaluate their status, offer assistance, and address inquiries.
Manage a diverse patient panel while adhering to NCQA standards for complex case management.
Communicate updates to the care team and family members, delivering tailored education and resources.
Act as a liaison with dialysis facilities, conveying care plan updates to facilitate smooth transitions in care.
Maintain accurate program software records and generate reports using Strive data platforms.
Provide culturally sensitive care to meet the diverse needs of patients.

3+ years experience in nephrology, ER, ICU, cardiac, or ambulatory complex case management nursing
Certified Case Manager (CCM) certification

Experience: 3+ years combined of related education, experience, or certification.

Education: Associate’s degree from an accredited school of nursing.
