Prepares patient for visit with provider by completing the Advanced Standard room process.
Enters the chief complaint, takes patient vital signs, and reviews patient history and allergies.
Completes a thorough medication reconciliation.
Works with the Encounter Guide to address open care gaps such as fall risk and depression screening.
Rechecks elevated blood pressures and completes necessary documentation in Epic for yearly preventative visits.
Assists with in-office testing including drawing blood, completing mental status exams, and point of care testing.
Administers vaccines and other injections within scope of practice.
Schedules follow-up appointments as needed per office policy.
Collaborates with providers and clinical care team to monitor and coordinate delivery of care plan.

Experience: 6 months clinical healthcare experience, preferably in a primary care office.

Education: High School Diploma or GED; Diploma in Practical Nursing for LPN; Associate or Bachelor's Degree in Nursing for RN.

License & Certification:

Active LPN or RN license
Medical Assistant certification/registration (RMA, CCMA, CMA) or EMT/Paramedic acceptable.
