Job Responsibilities
- Ensure patients at Mitchell Cancer Institute receive quality and comprehensive coordinated care services.
- Promote timely access to appropriate care and decrease emergency room visits.
- Increase utilization of preventative care services and promote patients’ self-management.
- Act as an advocate, interpreter, educator, and counselor for oncology patients and families.
- Create and promote adherence to treatment and survivorship care plans in coordination with patients and families.
- Serve as the primary contact point for patients, care teams, families, payers, and community resources.
- Provide care and guidance to cancer patients from screening to survivorship.
- Monitor and track patient care, addressing unmet health and social needs.
- Develop care plans with patients and families, ensuring post-treatment survivorship plans.
- Evaluate adherence to care plans, monitor patient progress, and make adjustments as needed.
- Perform continuous assessments, planning, implementation, and evaluation of oncology patient care.
- Educate patients and families, ensuring seamless, continuous, and comprehensive care.
- Coordinate chemotherapy education and appointment processes prior to treatment initiation.
- Document patient teaching and respond to requests for information about treatment and resources.
- Support families during difficult decision-making times and coordinate emotional and care support.
- Facilitate communication between primary care and specialty providers to ensure co-management.
- Assist in identifying high-risk patients and coordinating necessary tests and procedures.
- Help patients overcome barriers to care including transportation and financial counseling.
- Refer patients to supportive care services such as nutritionists and counselors.
- Assist in the coordination of care with clinical trials for eligible patients.
- Participate in multidisciplinary consultations and serve as a liaison between patients and medical staff.
- Maintain regular and prompt attendance, adhering to defined work schedules and overtime requirements.

Requirements
Experience: Five years of professional nursing experience, three of which were in oncology, OR seven years of professional nursing experience, five of which were in oncology.
Education: Bachelor’s degree in nursing from an accredited institution or graduation from an accredited school of nursing meeting state standards.
License & Certification:
- Current licensure with the state of Alabama as a registered nurse
- Certification to administer chemotherapy within six months of employment
- ACLS certification within six months of employment
