Job Responsibilities

Ensure patients at Mitchell Cancer Institute receive quality and comprehensive coordinated care services.
Promote timely access to appropriate care and decrease emergency room visits.
Increase utilization of preventative care services and promote patients’ self-management.
Act as an advocate, interpreter, educator, and counselor for oncology patients and families.
Create and promote adherence to treatment and survivorship care plans in coordination with patients and families.
Serve as the primary contact point for patients, care teams, families, payers, and community resources.
Provide care and guidance to cancer patients from screening to survivorship.
Monitor and track patient care, addressing unmet health and social needs.
Develop care plans with patients and families, ensuring post-treatment survivorship plans.
Evaluate adherence to care plans, monitor patient progress, and make adjustments as needed.
Perform continuous assessments, planning, implementation, and evaluation of oncology patient care.
Educate patients and families, ensuring seamless, continuous, and comprehensive care.
Coordinate chemotherapy education and appointment processes prior to treatment initiation.
Document patient teaching and respond to requests for information about treatment and resources.
Support families during difficult decision-making times and coordinate emotional and care support.
Facilitate communication between primary care and specialty providers to ensure co-management.
Assist in identifying high-risk patients and coordinating necessary tests and procedures.
Help patients overcome barriers to care including transportation and financial counseling.
Refer patients to supportive care services such as nutritionists and counselors.
Assist in the coordination of care with clinical trials for eligible patients.
Participate in multidisciplinary consultations and serve as a liaison between patients and medical staff.
Maintain regular and prompt attendance, adhering to defined work schedules and overtime requirements.

Requirements

Experience: Five years of professional nursing experience, three of which were in oncology, OR seven years of professional nursing experience, five of which were in oncology.

Education: Bachelor’s degree in nursing from an accredited institution or graduation from an accredited school of nursing meeting state standards.
