Job Responsibilities
- Rounding and managing the clinical care of the population at the assigned SNF.
- Providing comprehensive primary care services to patients residing in SNFs.
- Prescribing medications and treatments for residents.
- Timely documentation of all patient care/encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- On-site on and on time for scheduled shifts.
