Job Responsibilities
- Monitor and document the effectiveness of treatment provided.
- Coordinate the admission process and determine appropriate post-discharge services with the interdisciplinary team (IDT).
- Coordinate and update Care Plan.
- Attend meetings if requested by guest or deemed appropriate by IDT.
- Participate in daily IDT meetings and provide clinical review as necessary.
- Complete assigned sections of Discharge Plan Instructions.
- Review clinical post-discharge instructions with guests upon discharge.

Requirements
Experience:
Education: Nursing Degree from an accredited college/university or completion of an approved LPN/LVN program.
