Job Responsibilities

Identify guest acuity and needs to determine specific care needs.
Communicate needs and expectations to families and responsible parties.
Facilitate the completion of Omnibus Reconciliation Act (OBRA) assessments.
Facilitate the completion of Prospective Payment System (PPS) assessments.

Required Skills

Core Skills

Current valid LPN license or temporary LPN license
3.0 MDS Certification or minimum of one year structured 3.0 training experience
Active working knowledge of the RAI process
Experience with completion of CAA worksheets and comprehensive care plans
CPR Certification
Minimum of one year LPN experience
