Job Title: Licensed Practical Nurse - Family Medicine, All Sites - LPN
Position: LPN providing nursing care under supervision
Job Type: Full Time
Pay: $26.00 - $38.55 / hour
Shift: Monday thru Friday, 4 10-hour shifts with a rotating day off

Job Responsibilities
- Provide nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor.
- Collect data through observation and communicate information to assist the RN and/or provider in patient assessment and care planning.
- Participate as a member of the health care team.
- Deliver care in accordance with the patient care plan, policies, and procedures of the organization.
- Possess excellent communication skills; mentor and instruct as needed.
- Participate on committees or projects, including quality improvement projects.

Preferred Skills
- One year LPN experience within the last five years
- Previous clinic experience

Requirements
Experience: One year of LPN experience working in an applicable setting required if graduation did not occur within the last two years; one year preferred within the last five years.
Education: Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program.
License & Certification:
- Current LPN license by applicable state requirements
- BLS (Basic Life Support) certification valid for X days post start date

- Daytime hours, flexibility may be required to meet staffing needs.
