Job Responsibilities
- Provide oversight and guidance to people enrolled in the IRIS SDPC option.
- Administer the Wisconsin Personal Care Screening Tool.
- Create person-centered plans of care.
- Provide personal care oversight to a group of participants in IRIS.
- Provide education and training for IRIS participants and care providers.
- Conduct required documentation and follow-up.
- Document assessments required by individual tools and Department of Health Services policies.
- Develop individual plans of care and ensure physician orders for care are obtained.
- Submit for Prior Authorization for personal care services.
- Comply with all Department of Health Services policies and SDPC guidelines.
- Complete collateral contacts with IRIS Consultants and Long-Term Care Functional Screeners.
- Complete other duties as assigned.
- May require driving 50% of the time during member home visits.
- Navigate stairs, exposure to different environments, and pets in members' homes.

Requirements
Experience: Minimum 2 years of experience in nursing with at least one year in home health.
Education: Associates Degree in Nursing
License & Certification:
- Current unrestricted license in Wisconsin as a Registered Nurse
- Valid Driver’s License
