Job Responsibilities
- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for a specific location managed.
- Conduct and delegate patient assessments and reassessments, updating care plans and interpreting patient needs.
- Manage the assignment of caregivers.
- Ensure delivery of care to all patients served by the location.
- Receive case referrals and review available patient information to determine home health or hospice needs.
- Ensure patients meet admission criteria and make decisions to admit patients to services.
- Assign appropriate clinicians to cases as needed.
- Instruct and guide clinicians to promote effective performance and quality home care services.
- Assist clinicians in setting immediate and long-term therapeutic goals and developing patient Plan of Care (POC).
- Monitor cases to ensure documentation compliance with regulatory agencies and third-party payers.
- Coordinate communication between team members, attending physicians, and caregivers.
- Establish location’s revenue and budget goals in conjunction with the Branch Director and Company Finance Department.
- Participate in sales and marketing initiatives.
- Supervise all clinical employees assigned to the specific location.
- Handle necessary employee corrective action and discipline issues fairly and objectively.
- Participate in the interviewing, hiring, training, and development of direct care clinicians.
- Evaluate clinician performance relative to job goals and requirements.
- Create and conduct regular staff education based on assessed education needs.
- Analyze situations, identify problems, evaluate alternative courses of action using Performance Improvement principles.
- Conduct review of Case Managers and clinical staff documentation for appropriateness of care and delivery.
- Responsible for QA/PI activities and data tracking for performance review and outcomes of care analysis.
- Provide direct patient care on an infrequent basis during emergencies.
- Act as Branch Director in his/her absence.
- Ensure compliance with external regulatory authorities and internal standards.
- Participate in performance improvement activities and maintain ongoing clinical knowledge.
- Maintain relationships with referral/community sources and participate in professional organizations.
- Conduct care-related programs.
Required Skills
Core Skills
- Registered Nurse with a current state license
- Management experience in home care or hospice
- Strong leadership and communication skills
Preferred Skills
- CPR certification
- Experience in performance improvement
- Experience with regulatory compliance
Requirements
Experience: Two years as a Registered Nurse with at least one year of management experience in a home care, hospice or equivalent environment.
Education: Graduate of an accredited School of Nursing.
License & Certification:
- Registered Nurse License
- Current CPR certification
- Valid driver's license and auto insurance
