Job Responsibilities

Conducts initial and ongoing assessments to determine patient needs for care coordination and discharge.
Develops a focused discharge plan, especially for high-risk patients.
Works closely with the clinical team and medical providers to implement the discharge plan.
Stays current with healthcare trends, regulations, and payer requirements related to patient care, discharge planning, and benefits.

Required Skills

Core Skills

Basic professional knowledge equivalent to a Bachelor's degree in nursing
Working knowledge of theory and practice within a specialized field
Experience in inpatient case management/discharge planning

Preferred Skills

BSN preferred
Case Management certification preferred within 3-5 years of hire

Requirements

Experience: 3-5 years related experience; prior experience with inpatient case management/discharge planning strongly preferred.

Education: ADN required; BSN preferred.

License & Certification:

Registered Nurse License - Current Maryland license or eligibility to obtain Maryland license
Case Management certification preferred within 3-5 years of hire
