Assists patients in the process of navigating post-acute care with a goal of creating a positive impact on patient outcomes and referral source satisfaction.
Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and metrics in the development of patient-centered transition plans.
Promotes quality and efficiency in the delivery of post-acute care.
Represents area branches in strategic relationships with health systems, hospitals, inpatient facilities, and physicians.

Basic technology skills, including operation of a mobile device
Understanding of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice

Experience: Two years of demonstrated field experience

Education: Graduate of an approved school of nursing, therapy, or social work
