Job Responsibilities

  • Perform OASIS Start of Care admissions assessments.
  • Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
  • Initiate the plan of care and necessary revisions.
  • Provide services requiring substantial specialized nursing skills.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Prepare clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
  • Coordinate services.
  • Inform personnel of changes in the condition and needs of the patient.
  • Counsel the patient and family/significant others in meeting nursing and related needs.
  • Participate in and present in-service programs.
  • Understand and adhere to established Agency policies and procedures.
  • Process orders and notify physicians of patient needs and changes in condition.
  • Complete certification/recertification orders and discharge summaries.
  • Determine the amount and type of nursing needed by each individual patient.
  • Refer patients requiring specialized skills to appropriate therapists (Physical Therapist, Speech-Language Pathologist, Occupational Therapist, Medical Social Worker).
  • Supervise and teach other nursing personnel.
  • Conduct patient care conferences on patients assigned to his/her care.
  • Participate in peer review and Quality Assessment and Performance Improvement as assigned.
  • Give total patient care as needed.
  • Take on-call duty nights, weekends, and holidays, as assigned.
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in accordance with Agency defined time frames.
  • Appropriately utilize ICD-10 codes.

Requirements

Experience: Three (3) years of home health nursing experience, preferred.

Education: Graduate of an approved school of professional nursing.

  • Currently licensed in the state(s) in which practicing
