Job Responsibilities

Perform OASIS Start of Care admissions assessments.
Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
Initiate the plan of care and necessary revisions.
Provide services requiring substantial specialized nursing skills.
Initiate appropriate preventive and rehabilitative nursing procedures.
Prepare clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
Coordinate services.
Inform personnel of changes in the condition and needs of the patient.
Counsel the patient and family/significant others in meeting nursing and related needs.
Participate in and present in-service programs.
Understand and adhere to established Agency policies and procedures.
Process orders and notify physicians of patient needs and changes in condition.
Complete certification/recertification orders and discharge summaries.
Determine the amount and type of nursing needed by each individual patient.
Refer patients requiring specialized skills to appropriate therapists (Physical Therapist, Speech-Language Pathologist, Occupational Therapist, Medical Social Worker).
Supervise and teach other nursing personnel.
Conduct patient care conferences on patients assigned to his/her care.
Participate in peer review and Quality Assessment and Performance Improvement as assigned.
Give total patient care as needed.
Take on-call duty nights, weekends, and holidays, as assigned.
Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in accordance with Agency defined time frames.
Appropriately utilize ICD-10 codes.

Requirements

Experience: Three (3) years of home health nursing experience, preferred.

Education: Graduate of an approved school of professional nursing.
